Description: Naming and framing complex problems, such as the technical errors associated with laparoscopic cholecystectomy, are important steps in addressing them. In addition, the adoption of a systems view of patient safety can help strengthen defenses against errors such as misidentification of the cystic duct. Using detailed medical illustrations and an interactive learning design, this toolkit focuses on the perceptual and technical errors that result in injuries to the common bile duct and other structures, and also outlines approaches, such as the critical view, for avoiding these errors. The toolkit also provides team communication tips and information on how surgical support teams, including nurses, can assess patient risk and prepare the OR appropriately for possible secondary procedures. It also outlines tips to help nurses draw on their powers of influence and persuasion to help a surgeon understand concerns during a procedure.
